The group operates across four countries — Germany, India, the UK and the UAE — and comprises two core business units: (1) Healthcare@Home / ambulatory and healthcare services; (2) AI-powered international healthcare recruitment. Why this role exists

At Pflegeunion (part of TERN Group), we run one of the largest at-home elderly care businesses in Germany and we are reimagining it around AI. Not AI bolted onto a legacy process, a few faster steps and the same cost structure, but the operation reimagined as it would be built today, with AI at the core of every function that isn't hands-on care at the bedside.

We own and operate the care business ourselves. That distinction is the whole point: when we rebuild a process, we get to change how the operation actually runs, not just ship a feature and hope someone adopts it.

 

This is not a plan on a slide - the work is underway and running in the live operation today. A voice agent answers inbound calls around the clock, on every channel, and converts enquiries into clients. An AI coach trains and onboards our international caregivers in German, taking them through real on-the-job scenarios until they are job-ready. Alongside them, agentic workflows have started taking over parts of the back office, from documentation to same-day re-staffing.

There is far more still to build than we have built. We think the people best placed to build it are the ones who learned to work with AI from the start, rather than adapting to it later.

 

What you'll do

  • Own a real problem, not a side quest. You'll be given a genuine process in the business — something schedulers, recruiters or caregivers do every day — and asked to work out how it should run if AI did the heavy lifting. You'll have a senior PM alongside you, not above you.
  • Go and see the work. Sit with the people who run the operation. Listen to real calls. Watch a day of visits get planned and then re-planned when reality intervenes. Almost every good idea here starts there.
  • Build it yourself. We expect prototypes, not specs. Getting something rough in front of a caregiver or a scheduler within days is normal here, and it's the fastest way to learn what actually works.
  • Find out whether it worked. You'll define what success looks like before you build, then measure it honestly — including when the answer is that your idea didn't land.

 

What we're looking for

  • You build with AI. Either you've shipped something where AI is the core of the experience — a project, a tool, an agent, a product — or you build with AI development tools (Claude Code, Cursor, or similar) as a matter of routine. This is the requirement that actually matters to us.
  • You finish things. A rough prototype that real people used beats a polished idea that never left your laptop. Send us the former.
  • Fluent German and English. You'll be talking to caregivers, clients and families in German, and working with a distributed team in English. Both need to be genuinely fluent.
  • You're curious about how things really work. This business is messy, regulated and human. The interesting problems are hidden inside the details, and you'll only find them by asking a lot of questions.

 

Nice to have, not required

  • Any exposure to healthcare, home care, or another regulated operational business
  • Familiarity with ambulante Pflege, SGB XI, Pflegegrade, Leistungskomplexe, Tourenplanung, Abrechnung, MD audits — or the willingness to learn it fast
  • Experience with voice agents, LLM evaluation, or agent orchestration
  • A degree in anything. We genuinely don't mind which subject.
